 Basic Information Change

(1) Eligible applicants
秒速pk10To change the beneficiary, the insured or the group customer after obtaining the consent(the authorization) of the insured should submit an application, and the application documents must be signed by the insured as confirmation. For group members who have applied for long-term insurance (universal), the application for the collection of dimission severance or pension needs to be put forward by the insured, sealed by the applying group customer as confirmation. Similarly, for group members who have applied for long-term insurance (universal), the application for partial collection or transfer of the account which belongs to individuals should be put forward by the insured, sealed by the applying group customer as confirmation. For all other conservation items, the applicant will be the group customer.

(2) Required documents
General documents required for handling conservation matters (for specific information, please refer to the List of Application Materials for Group Insurance Conservation Items); application form (different applications will be used for different products); list of the insured (different lists will be used for different products); photocopy of valid identity card (including: valid identity card, military identity card, passport or other relevant proof documents issued by the registered permanent residence management department of the public security organs) .

(3) Filling and sealing of application documents
秒速pk10Application documents should be filled out by the eligible person. All documents that need to be signed by the insured must be personally signed by the insured. If the (joint) insured is a non-adult, application documents must be signed by his or her statutory guardian as confirmation; group customers must use in ink pen or signing pen to “correctly” and “completely” fill in each item. In case of alteration, the customer must cover the altered part with the seal of the agent of the insurance application organization; in case of serious alteration, new documents must be filled out; except that only Health Declaration for Insured under Group Insurance does not need to be affixed with the official seal of the applying organization, all the other application documents (including lists) must be affixed with the official seal of the applying organization, and the name of the official seal must be consistent with the name of the applying organization; one application can only correspond to the application for conservation items under one group policy.

(4) Others
秒速pk10Group customers must submit the original copy of application documents to apply for conservation service items; where the list of the insured is over 30, please provide an electronic list; customers in long-term insurance group who apply for the collection of dimission severance/pension must submit the original copy of proof of quitting of job (resignation form or proof of resignation issued by other labor supervision department) or proof of retirement (retirement certificate specially issued by the State) as well as the personal policy insurance document of the insured, and upon examination and verification by the company, the original copy will be returned, while a photocopy will be retained; all changes must comply with the minimum sum assured/premium provisions of the company.
